RIP. What a terrible thing to happen.

"Flash bangs" are stun grenades, used to disorient people in a small room or enclosed area with a bright flash of light and loud percussive noise prior to going in to get them. I used them several times during my Army days, like most weapons they are dangerous to use and, unfortunately, can be lethal in the wrong circumstances. From what I gather, the Officer was attempting to deactivate an unused one when it went off.
